Preparation for Startup

Please fill out this form in preparation for your start up (before the unit is installed) and to request a date for remote startup. This form provides us with preliminary information in order to begin the process for your startup. After you fill out this form, the next step is for you is to fill out the Pre Startup Checklist (https://mitsairconditioning.formstack.com/forms/pre_startup_checklist). Pre Startup Checklist is mandatory and must be completed at least 10 business days prior to the proposed startup date.
